As a Home Health RN Case Manager, you’ll manage a caseload of patients, providing high-quality, patient-centered care. You’ll coordinate with an interdisciplinary team to develop and implement individualized care plans, ensure visit utilization is efficient, and maintain the highest standards of clinical documentation and compliance.
