How you'll contribute

A Case Manager (RN) who excels in this role:

· Responsible for overall management of the care coordination of observation/inpatients in collaboration with the care team.

· Also responsible for appropriateness of care and resource utilization, as well as, continued stay reviews

· Responsible for providing education as an adjunct to care team, particularly in regards to chronic disease management.

What we're looking for: Applicants should have a current VA or compact RN license.

Additional requirements include:

· BSN preferred

· Basic Life Support certification is required within 30 days of hire.

· ACLS, PALS & ASLS preferred
