Salary: Starting at $42 per visit


Schedule: Flexible schedule, Monday through Friday

Job Description:

We are currently seeking a Registered Nurse (RN) to join our team as a Home Health Case Manager in the Denver Metro Area. As a Case Manager, you will be responsible for providing compassionate and comprehensive home health care services to our patients.

Responsibilities:

• Conduct initial patient assessments to develop personalized care plans.
• Provide skilled nursing care to patients in their homes.
• Monitor and evaluate patient progress, adjusting care plans as necessary.
• Coordinate with other healthcare professionals and community resources to ensure continuity of care.
• Educate patients and their families about their conditions and treatment plans.
• Document all patient care activities accurately and in a timely manner.

Qualifications:

• Current Colorado RN license.
• Minimum of 1 year of experience in home health or a related field.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and manage time effectively.
• Valid driver's license and reliable transportation.

Compensation: $42.00 - $67.00 per hour
